BiEst or estradiol?

The short answer
BiEst combines estriol with estradiol. A custom ratio does not establish better symptom relief or lower risk than an approved estradiol product, and bioidentical is not a synonym for compounded.

What the evidence shows
ACOG recommends approved menopausal hormone formulations when suitable options exist and notes the lack of high-quality safety and efficacy evidence for custom-compounded therapy. That leaves room for a specific formulation need, without establishing that a blend is inherently safer.

Ask why the proposed preparation fits your symptoms, whether an approved option meets that need and how treatment will be followed. Confirm total strength and application instructions rather than choosing a product from a ratio alone.

Does adding estriol make systemic estrogen risk-free?
No. A second estrogen does not establish a lower-risk regimen. The health-history review and, when relevant, endometrial-protection plan still matter.
