Erectile health for gay and bisexual men: care on your terms.
In this article
Erectile health care should make room for the sex you enjoy. For gay and bisexual men, that can mean reliable erections for penetration, more ease during oral sex or mutual masturbation, or feeling comfortable exploring a new connection. Your preferences deserve to lead the conversation.
There is no single gay or bisexual sex life. You may have one partner or several, be dating or partnered, enjoy penetration or prefer sex without it. None of those details can be assumed from your orientation. Start with what matters to you.
Define what you want help with
“Better erections” can mean very different things. Make your goal concrete enough that you and a clinician can discuss it:
- “I want to stay firm enough for insertive sex when I choose it.”
- “Erections are less reliable during mutual masturbation, and I miss that part of sex.”
- “I enjoy receptive sex, but I am worried about what a partner thinks when I am not hard.”
- “I want to understand why erections differ when I am alone and with someone else.”
You do not need an erection for every sexual activity. If you enjoy receptive sex, giving oral sex, or touch without being erect, there is no obligation to change that. Equally, wanting a more reliable erection is a valid goal even when penetration is not part of your sex life.
Describe activities, not just labels
“Top,” “bottom,” “versatile,” or “side” may be helpful shorthand if you use those words. They do not tell a clinician everything about sensation, comfort, desire, or the activities you want to enjoy.
Be specific about when firmness changes. During oral sex? When putting on a condom? Before penetration? After changing position? Mention pain, altered sensation, or difficulty reaching orgasm as separate concerns rather than assuming an erection medicine will address all of them.
A thorough ED assessment asks about desire, erections, ejaculation, and climax alongside medical and mental health history. Your actual experience belongs in that history. NIDDK: what an ED assessment covers.
For bisexual men, a current partner’s gender does not change your identity. Describe differences between situations if they are useful, without treating any one encounter as a test of who you are attracted to.
Keep physical and emotional contributors in view
Health conditions affecting circulation, nerves, or hormones can contribute to ED, as can medicines, stress, and anxiety. Your sexual orientation does not replace an evaluation of those factors. NIDDK: causes of erection difficulties.
There is also room to discuss experiences outside the bedroom. A study of young men who have sex with men found an association between internalized stigma and lower overall sexual satisfaction. It was a cross-sectional study, so it cannot show that stigma caused an individual’s erection difficulty. Li and colleagues, original research.
If judgment, body comparison, or pressure to play a certain role feels relevant, say so. You can ask for support that respects your orientation and relationship agreements while keeping your specific concern in focus.
Where tadalafil belongs
Tadalafil helps support erections in response to sexual stimulation. It does not create desire or an automatic erection. Daily and as-needed regimens are options for ED. Tadalafil prescribing information.
Think about the practical fit: your routine, how you like to plan, and whether the concern is erection reliability, interest, comfort, or a combination. Our daily versus on-demand tadalafil guide explains the scheduling differences. If desire is the main concern, erections without desire starts with that question instead.
An erection medicine does not define your sexual role or require you to penetrate. You can keep the activities and boundaries you enjoy while discussing the specific change you would like help with.
Know the safety rules that apply to everyone
Never combine tadalafil with nitrates, nitrite “poppers,” or riociguat: the combination can dangerously lower blood pressure. This matters regardless of orientation. Tell the prescriber about every medicine, supplement, and recreational substance you use so they can assess interactions. Tadalafil patient information.
Common side effects include headache, indigestion, and back pain. An erection lasting more than four hours needs emergency care. Stop tadalafil and seek immediate medical help for sudden vision or hearing changes. It does not protect against sexually transmitted infections. Prescribing information.
Bring your own definition of progress
Before an assessment, finish this sentence: “I would like sex to feel more…” Then add a concrete example. More reliable during penetration, more comfortable talking with partners, or more enjoyable without monitoring your erection are different goals worth naming.
