I want sex. Why isn’t my body cooperating?
In this article
You can want sex and still have trouble getting or keeping an erection. Desire and erectile response involve different parts of sexual function. Recurrent difficulty deserves attention, and it does not automatically mean that attraction has faded or that a partner is doing something wrong. Physical health, medications and emotional factors can all contribute. NIDDK: symptoms and causes.
The useful question is what happens between wanting the experience and having it. That gives you a clearer starting point than deciding you have somehow failed at sex.
Describe the part that is difficult
Try to separate interest, firmness, sensation and orgasm. “I look forward to sex, but lose firmness when we change activities” is more informative than “my libido is bad.” So is “I can get an erection, but I spend the whole time worrying about keeping it.”
Before a conversation with a clinician, note:
- Whether getting an erection, keeping it, or both are difficult.
- Whether the pattern happens alone, with a partner, or in particular situations.
- When it began, whether it changed suddenly, and whether there is pain or curvature.
- Any change in medicines, health, sleep, stress or substance use around that time.
- What you would like to enjoy more comfortably or confidently.
These notes prepare you for a discussion. They are not a home diagnostic test. A clinician may use medical, sexual and mental-health history, examination and selected tests to understand the pattern. NIDDK: diagnosis.
Wanting sex does not remove physical contributors
Blood-vessel disease, diabetes, nerve problems, certain medications and hormonal conditions can contribute to ED. Anxiety and stress may also be involved, sometimes alongside a physical issue. The fact that something works on one occasion does not explain the whole pattern. NIDDK: causes.
This is worth taking seriously even if you feel young and active. Our guide to ED in your 30s and 40s focuses on getting useful answers without treating age as the diagnosis.
If a prescription seems connected to the change, bring it up rather than stopping it. For example, our antidepressant and sexual-health guide separates desire, erections and orgasm so the conversation stays specific.
Where tadalafil and sildenafil fit
Oral ED medicines can help blood flow to the penis. They are among several treatment options, which also include addressing underlying health conditions, counseling, devices and other prescribed therapies. NIDDK: treatment.
Tadalafil needs sexual stimulation. Its daily schedule separates taking the medication from the occasion, while an on-demand prescription is used before anticipated sex. It does not create desire or cure every cause of erection difficulty. Tadalafil prescribing information.
Start with the decision that matters to you:
- Daily or on-demand tadalafil if medication timing is the main question.
- Sildenafil or tadalafil if you want to compare different routines.
- What to check when ED medication is not working if you have already tried treatment.
Low desire calls for a different assessment. PT-141 and ED medications explains the distinction without treating them as interchangeable or assuming you need a combination.
Take pressure out of the next encounter
You do not need to make the next evening a pass-or-fail test. You can tell a partner, “I am interested in you. My erections have been less predictable, and I am looking into it.” Keep the conversation about the experience you both want rather than asking either person to prove attraction.
It may help to agree that an erection does not have to determine whether the encounter continues. Choose activities you enjoy, allow a change of pace, and avoid repeatedly checking whether your body is doing what you expected. Counseling may be useful when stress or anxiety is part of ED. NIDDK: counseling and care.

Use the assessment to look beyond the symptom
ED can be a marker of cardiovascular risk. The Princeton IV consensus recommends evaluating that connection rather than treating an erection response as proof that the heart and blood vessels are healthy. That does not mean every erection difficulty indicates heart disease. Princeton IV.
Our tadalafil and heart-health article explains the research separately from the established ED benefit.
Before taking tadalafil, disclose all medicines and recreational substances. Nitrates, poppers and riociguat are incompatible with it; cardiovascular, kidney and liver conditions also matter. The safety guide explains the main checks. Prescribing information.
If erections are what you want help with, explore the daily tadalafil plan. Bring the pattern you have noticed and the experience you want to improve. Those details make the next conversation more useful.
