What the research shows
Both once-daily doses beat placebo on erectile function and intercourse success.
The detail
In a 12-week multicenter randomized, double-blind, placebo-controlled trial, 268 men were assigned in a 1:2:2 ratio to placebo, tadalafil 5 mg or tadalafil 10 mg taken once daily. Change from baseline in the International Index of Erectile Function erectile-function domain was 0.9, 9.7 and 9.4 respectively; successful penetration on Sexual Encounter Profile question 2 rose 11.2, 36.5 and 39.4 points; successful intercourse on question 3 rose 13.2, 45.5 and 50.1 points. All comparisons against placebo were significant at p less than 0.001.1
84.5 percent on tadalafil reported improved erections against 28.3 percent on placebo.
The detail
In the same trial, 28.3 percent of men on placebo reported improved erections at endpoint compared with 84.5 percent on tadalafil 5 mg and 84.6 percent on 10 mg, and 8.3 percent on placebo scored in the no-erectile-dysfunction range of 26 to 30 on the erectile-function domain compared with 51.5 percent and 50.5 percent on the two once-daily doses.1
Lower once-daily doses also beat placebo on all three co-primary endpoints.
The detail
In a 24-week randomized, double-blind, placebo-controlled study at 15 United States centers, following a four-week treatment-free run-in, tadalafil 2.5 mg and 5 mg taken once daily significantly improved erectile function against placebo on all three co-primary endpoints: the erectile-function domain and the mean per-patient percentage of yes responses to Sexual Encounter Profile questions 2 and 3.2
Across the labeled programme, the effect did not diminish over six months.
The detail
The FDA-approved labeling describes a once-daily programme of two 12-week trials and one 24-week trial involving 853 patients in total, and reports that in the six-month double-blind study the treatment effect did not diminish over time.3
Median time before stopping was longer on daily tadalafil than on-demand sildenafil.
The detail
In an open-label randomized study in 770 men new to PDE5 inhibitors, median time to discontinuation of assigned treatment was 130 days for tadalafil 5 mg once daily and 67 days for sildenafil 50 mg on demand, with Kaplan-Meier discontinuation estimates of 52.2 percent and 66.7 percent respectively.4

Daily dosing has not been shown to work better than on-demand dosing.
The detail
Once-daily dosing has not been shown to work better than on-demand dosing. In the randomized comparison above, improvement in the erectile-function domain did not differ between tadalafil once daily, tadalafil on demand and sildenafil on demand, least-squares means 9.4 to 10.0, p equals 0.359. In a pooled comparison of 17 on-demand and 4 once-daily placebo-controlled studies covering 4354 men, the authors concluded that no clinical population appeared to benefit overwhelmingly from one dosing regimen over the other, and that comparison was descriptive rather than a randomized head-to-head. The case for daily dosing is fit and persistence, not superior efficacy.45
More men preferred on-demand dosing, and the scheduled arm was not this dose.
The detail
Preference is real, and it splits. In an open-label crossover study of 4262 men in 14 European countries, 57.8 percent preferred tadalafil 20 mg on demand and 42.2 percent preferred the scheduled regimen. That scheduled arm was 20 mg three times per week, not the low once-daily dose prescribed here, so it is preference evidence about a related schedule rather than this one.6
Nothing here shows the effect persists after the medication is stopped.
The detail
The once-daily trials ran 12 to 24 weeks and the labeled programme totals 853 patients. That establishes an effect that held up over six months of continuous dosing. It does not establish that continuous exposure restores erectile function that persists after the medication is stopped, and it does not describe outcomes beyond those durations.123
It does not cure erectile dysfunction, and it does nothing without sexual stimulation.

A daily schedule does not change what the drug does. The endpoints remain self-reported intercourse success and questionnaire scores. The FDA-approved labeling states that tadalafil has no effect in the absence of sexual stimulation, and the patient labeling states that the medication does not cure erectile dysfunction and does not increase sexual desire.3
