What the research shows
81 percent of men on tadalafil reported improved erections, against 35 percent on placebo.
The detail
In an integrated analysis of five randomized, double-blind, placebo-controlled 12-week trials in 1112 men of mean age 59 with mild to severe erectile dysfunction of various causes, tadalafil 20 mg taken as needed produced a mean improvement of 7.9 points in the International Index of Erectile Function erectile-function domain from baseline, 75 percent of intercourse attempts were completed successfully on the Sexual Encounter Profile question 3, and 81 percent of men reported improved erections at endpoint compared with 35 percent on placebo.1
At 36 hours after a dose, 59.2 percent of attempts succeeded versus 28.3 on placebo.
The detail
In a multicenter randomized, double-blind, placebo-controlled trial in 348 men in Europe and the United States, participants were assigned to attempt intercourse approximately 24 or 36 hours after a dose. At 36 hours, 59.2 percent of attempts were successful on tadalafil 20 mg versus 28.3 percent on placebo; at 24 hours, 52.9 percent versus 29.1 percent.2
The approved as-needed programme runs to 22 trials and over 4000 patients.
The detail
The FDA-approved labeling describes an as-needed programme of 22 clinical trials of up to 24 weeks involving over 4000 patients, including seven randomized, multicenter, double-blind, placebo-controlled, parallel-arm 12-week studies in the general erectile-dysfunction population, with three co-primary outcomes: the erectile-function domain of the International Index of Erectile Function and Sexual Encounter Profile questions 2 and 3.3
Tadalafil 10 mg sat mid-pack on efficacy with the lowest overall adverse-event rate.
The detail
In a network meta-analysis of 82 randomized trials covering 47,626 patients, tadalafil 10 mg had intermediate efficacy among the starting doses compared and the lowest overall rate of adverse events, while sildenafil 50 mg had the greatest efficacy and the highest overall adverse-event rate.4
Men stayed on tadalafil on demand longer than on sildenafil before stopping.
The detail
In an open-label randomized study in 770 men new to PDE5 inhibitors, median time to discontinuation of the assigned treatment was longer than 168 days for tadalafil 10 mg on demand and 130 days for tadalafil 5 mg once daily, compared with 67 days for sildenafil 50 mg on demand. Reported reasons for stopping that differed significantly between groups included lack of duration of erection and time constraints from a short window of action.5
What it does not show
At 36 hours, roughly four in ten attempts did not succeed.
The detail
The 36-hour figure is a measured window of responsiveness, not a promise about an erection. In the trial that produced it, men were instructed to attempt intercourse at approximately 36 hours after dosing and 59.2 percent of those attempts succeeded, which means roughly four in ten did not. Nothing in that design establishes 36 hours of continuous effect.2
Placebo attempts succeeded too, and 35 percent on placebo reported improved erections.
Tadalafil does not cure erectile dysfunction and does not increase sexual desire.
The detail
The endpoints are self-reported intercourse success and questionnaire scores over 12 to 24 weeks. They do not establish an effect on desire. The FDA-approved labeling states that tadalafil has no effect in the absence of sexual stimulation, and the patient labeling states plainly that the medication does not cure erectile dysfunction and does not increase sexual desire.13
Superiority over the alternatives is not established; only time on treatment differed.
The detail
Superiority over the alternatives is not established. In the randomized comparison of tadalafil on demand, tadalafil once daily and sildenafil on demand, improvement in the erectile-function domain did not differ between the three arms, least-squares means 9.4 to 10.0, p equals 0.359; what differed was how long men stayed on treatment. The labeling also states that combinations of tadalafil with other PDE5 inhibitors or other erectile-dysfunction treatments have not been studied.35

Do not use if you have
- Nitrates in any form, an exclusion; the labeling advises that at least 48 hours pass after the last dose before nitrate administration is even considered in an emergency
- Riociguat, a guanylate cyclase stimulator, a labeled contraindication for the same reason and an exclusion as well
Serious risks
- An erection lasting longer than four hours needs emergency care
- Sudden loss of vision in one or both eyes, or a sudden decrease or loss of hearing, means stopping the medication and seeking medical attention immediately
- Tadalafil has a long terminal half-life, around 17.5 hours in healthy subjects, so the window in which an interaction can occur is long as well

- Headache and indigestion are reported most often, followed by back pain, muscle aches, nasal congestion, flushing and limb pain
- Back pain and muscle aches generally began 12 to 24 hours after a dose and typically settled within 48 hours
